The Agency provides opportunity for patients, family members, or legal representatives to call with a grievance or complaint of our services or staff on a 24-hour per day basis.  Resolution will be documented of the complaint.

Should you have any grievance, related to discrimination, all complaint calls received during regular business hours will be directed to the Administrative/Executive Director, Agency Director, Supervising Nurse or a designated available RN. Grievances related to discrimination issues will be directed to the Compliance Officer @ 800.213.4732, ext 2005.
